At Northeast Primary Care, we recognize how modern healthcare has changed; it has become increasingly rushed and fragmented, often limiting patients' ability to discuss all their concerns at one time, receive clarity about a diagnosis, or have the continuity of a primary care provider who truly knows them. What's even more challenging is when patients often have to wait months, if not years, to see their primary care provider or a specialist and have their medical needs addressed.
With our experience caring for adults and children in hospitals and outpatient settings for over a decade, we believe primary care requires something increasingly rare- increased time with their provider and developing the patient-provider relationship. Patients deserve better, and Northeast Primary Care was developed.
At Northeast Primary Care, we are passionate about delivering high-quality healthcare services that make a difference in people's lives. Our focus is on promoting wellness, preventing illness, managing acute conditions, and incorporating thoughtfulness into your unique medical needs. Through our concierge, membership-based primary care model, our goal is to ensure your care is met with 24/7 access to a practitioner, same or next-day appointments, and you receive care provided directly at your home or through Telehealth (office location pending).
By requiring a small membership fee, it allows us to maintain a smaller patient-to-provider ratio (300 patient limit/provider) to allow you more accessible, high-quality, and personalized healthcare. At Northeast Primary Care, there are no long wait times or waiting rooms. Instead, we can come right to you and refill prescriptions often within the day or, very rarely, the following day.
Our goal is to forge a trusting, collaborative, and supportive relationship with you that is long-term, personalized, consistent, and dependable. Jennifer DiBenedetto, PhD, APRN, FNP-C
Hi! I am the founder of Northeast Primary Care, PLLC. As a family nurse practitioner, I am dedicated to delivering compassionate, personalized primary care to families and busy professionals who value convenient, high-quality healthcare. Through a concierge primary care model that offers Telehealth and in-home wellness and sick visits, I offer un-rushed appointments, proactive care, and direct access that fit seamlessly into demanding schedules. By building trusted relationships and focusing on preventive, whole-person care, I help patients and their families stay healthy, supported, and confident—without the stress of traditional office visits.
As a PhD-prepared Nurse Practitioner, I strive to bring the latest research into everyday care, using evidence-based practices and trusted clinical guidelines to ensure thoughtful, high-quality treatment. With my nearly 15 years of experience in adult critical care and acute care, pediatric and adult urgent care and primary care, in addition to my background in complementary and alternative therapies, I am confident in managing your care at all stages of wellness and illness. I look forward to working with you!

Our primary care program emphasizes preventive care, routine check-ups, and early intervention to keep you healthy and address any health concerns proactively.
Home Visits
We offer home visits to make your primary care needs accessible. We will bring all the supplies to your home and allow you to receive medical care during wellness and sick visits
Urgent Care
In the event you need urgent care, our dedicated team is available to provide urgent medical attention and care when you need it most. You can receive IV fluids, medications, suturing, or other services outlined in our membership.
Our Program Services
Acute Illness Visits
Easy and prompt care for illnesses such as colds, rapid testing, UTIs, and minor injuries.
Health and Wellness Coaching
Our team is dedicated to your health and wellness goals. We will work with you on ways to optimize your health, whether it be vaccines, cancer screenings, dietary and nutritional guidance, or lifestyle coaching to help you achieve optimal wellness.
Preventative Care
Evidence-based care for conditions such as high blood pressure, diabetes, renal disease, or other chronic conditions
Membership Options
At Northeast Primary Care, we have several options for you to choose from when joining our team. We currently charge $450.00/mo for individuals and $900.00/mo for couples and families to join our team as a standard rate. This fee grants you a free comprehensive annual wellness visit (60 minutes), 24/7 direct access to a practitioner, unlimited prescription refills, access to home visits within the greater Worcester or Boston area, and more. If you have a high deductible health plan and have an HSA/FSA, part of your membership fee can be covered. A good-faith estimate for the year for the cost of all services will be provided to you in advance of signing onto the team.

1. Membership+Insurance (concierge). We are partnering with some insurance providers to bill your insurance for the cost of sick visits or other aspects of care you may require (Blue Cross Blue Shield is currently pending and anticipated to be accepted in June 2026). If you have other insurance, we are happy to send you a superbill for all PPO plans as an out-of-network provider.
2. Membership + self-pay (direct primary care). If you have an HMO plan or we do not accept your insurance, you can still become a member and self-pay for all additional primary care visits or services at wholesale costs. We can store your insurance information on record to submit prescriptions, claims for labs/diagnostics, or referrals, but you will self-pay for all sick visits and other costs.
3. Membership Sliding Scale (with insurance or self-pay). You can join our team at reduced pricing based on your income level and location. During your complimentary meet and greet, we can discuss if you qualify for reduced pricing.
4. Membership Only (no additional fees). With a membership-only plan, your membership fee is higher than the standard rate, but it includes 24/7 access to a practitioner, up to 4 sick visits a month with testing supplies included, and unlimited communication and treatment team meetings with other providers. This option is best suited for patients requiring frequent visits from their primary care provider or who have complex medical needs.
A La Carte Urgent Care Service
If you are not a member and would like to be seen for a sick visit, we are happy to provide a one-time urgent care price. We can either drive to your home or see you via Telehealth, depending on your needs. Please call and leave us a voice message about what you're looking for, and we'll get back to you between other patients. We accept all major debit/credit cards or Venmo.
Flat Rate = $200.00.
Family members of Northeast Primary Care Members = $100.00
If you live outside of Worcester County, please allow us additional time to see you that day to accommodate other patients' schedules. If you live outside a 100-mile radius, we may be able to see you via Telehealth.
We are only able to serve patients residing in Massachusetts at this time.
